Introdução: As crises febris (CFs) são o distúrbio neurológico mais comum na infância e uma causa frequente de atendimentos em emergências pediátricas. Classificadas em Simples, geralmente benignas, e Complexas, que podem estar associadas a risco de lesões intracranianas, atrasos no desenvolvimento ou epilepsia. Algumas CFs podem evoluir para Estado de Mal Epiléptico Febril (EMEF), uma condição grave. Não há preditores definitivos para essas complicações. Objetivos: Descrever as características clínicas e epidemiológicas das crises febris, explorar a relação com recorrência e EMEF, e avaliar o manejo clínico dos pacientes internados. Métodos: Estudo transversal, retrospectivo e analítico, com levantamento de prontuários de crianças de até 5 anos internadas por CFs entre 2017 e 2023. Analisaram-se dados de eletroencefalograma, exames de imagem, hemoglobina e sódio sérico. Resultados: Dos 322 pacientes, 59% eram do sexo masculino, com mediana de 16 meses. Em 72%, era o primeiro episódio de CFs, com etiologia mais comum em infecção de vias aéreas. Em 92,9% dos casos, a crise ocorreu nas primeiras 24 horas de febre. Das crises, 53,4% foram Simples e 46,6% Complexas. EMEF ocorreu em 14,9% dos casos. Não houve correlação significativa entre prematuridade, intensidade da febre, histórico familiar, exame neurológico prévio, cor/etnia e sexo com a recorrência de CFs. Contudo, frequência escolar, eletroencefalograma alterado e CFs Complexas mostraram relação com a recorrência. Anemia e alterações no eletroencefalograma foram mais frequentes nos casos de EMEF. Conclusão: As crises febris são majoritariamente benignas e breves, com a recorrência sendo o único desfecho adverso significativo. A principal função médica é tranquilizar as famílias quanto ao bom prognóstico destas crises
