RELAÇÃO ENTRE INFERTILIDADE E DEFICIÊNCIA DE VITAMINA D NA PACIENTE COM SOP: UMA REVISÃO

Relationship between infertility and vitamin D deficiency in patients with PCOS: a review

OBJETIVO: Avaliar a relação entre infertilidade e deficiência de vitamina D na paciente com SOP (Síndrome dos Ovários Policísticos) METODOLOGIA: Revisão narrativa realizada a partir de busca na base de dados PubMed utilizando os descritores “(infertility) AND (vitamin D deficiency) AND (polycystic ovary syndrome)”. Foram encontrados 40 artigos e após a aplicação dos critérios de exclusão, foram selecionados 8 artigos. RESULTADOS: Três estudos observaram a prevalência da deficiência de vitamina D em mulheres inférteis com SOP. No primeiro, Krishna Mogili et al avaliaram a presença de hipovitaminose D em 90,6% da amostra. No segundo, Demet Kokanalı et al observaram que a deficiência de vitamina D é mais comum em mulheres inférteis com SOP do que em mulheres férteis sem SOP. No último, Raziah Firouzabadi et al observaram que 83% das pacientes com SOP apresentaram deficiência de vitamina D. Samantha Butts et al., realizaram dois ensaios clínicos randomizados; no primeiro as mulheres com SOP e deficiência de vitamina D eram menos propensas a ovular, enquanto, no segundo, não foi observada associação significativa. Quatro dos estudos avaliaram a correlação entre vitamina D e as manifestações da SOP, incluindo fertilidade, ao comparar parâmetros pré e pós suplementação vitamínica. Majid Dastorani et al e Mohamad Irani et al demonstraram que a suplementação de vitamina D, o primeiro em mulheres inférteis com SOP e o segundo em mulheres com deficiência de vitamina D com SOP, reduziu significativamente o HAM (hormônio antimulleriano) sérico sugerindo melhora na foliculogênese. Raziah dehghani Firouzabadi et al avaliaram a eficácia da suplementação de vitamina D em mulheres inférteis com SOP e observaram melhoria na regulação de anormalidades menstruais, maturação do folículo e infertilidade. Mohamad Irani et al avaliaram mulheres com deficiência de vitamina D e SOP; nenhuma delas estava sob tratamento de fertilidade e 11,11% conceberam após a suplementação vitamínica, enquanto nenhuma concebeu depois do placebo. CONCLUSÃO: A maioria dos estudos endossam a relação entre deficiência de vitamina D e infertilidade na paciente com SOP e metade deles demonstram que suplementar vitamina D pode ser uma ferramenta terapêutica no manejo da infertilidade na SOP. São necessários mais estudos com amostras expressivas a fim de elucidar melhor essa relação.

OBJECTIVE: To evaluate the relationship between infertility and vitamin D deficiency in a patient with PCOS (Polycystic Ovary Syndrome). METHODS: Narrative review performed from a search in the PubMed database using the descriptors “(infertility) AND (vitamin D deficiency) AND (polycystic ovary syndrome)”. Forty articles were found and after applying the exclusion criteria, 8 articles were selected. RESULTS: Three studies looked at the prevalence of vitamin D deficiency in infertile women with PCOS. The first one, Krishna Mogili et al evaluated the presence of hypovitaminosis D in 90.6% of the sample. The second, Demet Kokanalı et al found that vitamin D deficiency is more common in infertile women with PCOS than in fertile women without PCOS. In the latter, Raziah Firouzabadi et al was observed that 83% of the patients with PCOS had vitamin D deficiency. Samantha Butts et al. performed two randomized clinical trials; in the first, women with PCOS and vitamin D deficiency were less likely to ovulate, while in the second, no significant association was observed. Four of the studies evaluated the correlation between vitamin D and PCOS manifestations, including fertility, when comparing pre- and post-vitamin supplementation parameters. Majid Dastorani et al and Mohamad Irani et al demonstrated that vitamin D supplementation, first in infertile women with PCOS and second in vitamin Ddeficient women with PCOS, significantly reduced serum AMH (anti-Mullerian hormone) suggesting improvement in folliculogenesis. Raziah dehghani Firouzabadi et al evaluated the efficacy of vitamin D supplementation in infertile women with PCOS and observed improvement in the regulation of menstrual abnormalities, follicle maturation and infertility. Mohamad Irani et al evaluated women with vitamin D and PCOS deficiency; none of them were on fertility treatment and 11.11% conceived after vitamin supplementation, while none conceived after placebo. CONCLUSION: Most studies endorse the relationship between vitamin D deficiency and infertility in patients with PCOS, and half of them demonstrate that supplementing vitamin D can be a therapeutic tool in the management of infertility in PCOS. More studies with expressive samples are needed in order to better elucidate this relationship.
