**INTRODUÇÃO: **A doença de Alzheimer (DA) é a principal patologia neurodegenerativa responsável por demência em idosos. Apesar de o sintoma cardinal ser a amnésia, há outros que, frequentemente, se associam. Assim, é importante a identificação dos sintomas não amnésticos mais comuns, pelo potencial auxílio diagnóstico permitindo um tratamento multiprofissional mais adequado ao quadro clínico do paciente. **MÉTODOS: **Realizou-se busca de artigos na PubMed, SciELO e Google Acadêmico, de 2010 a 2020, de acordo com as palavras-chave. Foram selecionados 54 artigos através de seus resumos. Aspectos relevantes sobre o tema foram encontrados em 33 desses, sendo utilizados para a construção deste trabalho. **DESENVOLVIMENTO**: O diagnóstico da DA é clínico, de forma que a anamnese e o exame físico neurológico mostram-se fundamentais. No início da doença, a função executiva (passível de avaliação pelo Mini-Mental, MoCa dentre outros) mostrou-se alterada em até 76% dos pacientes. Nos estudos analisados, foram aplicados o teste de trilhas B, teste de Stroop e a sequência de Luria, justificando o uso de pelo menos um desses em casos suspeitos de DA. A hiposmia também associou-se significativamente - em 2012, 2 estudos longitudinais de coorte e 30 transversais demonstraram maior frequência de hiposmia no Alzheimer. Em 2017, o artigo de N mais expressivo sobre o tema (mais de 700 participantes) chegou a sugerir a avaliação olfativa (através do “Sniffin’ Sticks Odor Identification Test”)  como parte suplementar do screening em suspeitas de DA, uma vez os resultados apontarem associação estatisticamente significativa entre déficit olfativo à DA e a Comprometimento Cognitivo Leve (CCL). Em fases mais avançadas, são comuns alterações nos testes da praxia, sendo a apraxia construtiva preditor de dependência mais rápida nesses pacientes. Com a deposição de peptídeo beta-amilóide neuronal, os pacientes apresentam também piora da qualidade do sono, com maior vigília e fragmentação do sono em até 60,60% dos doentes. A apresentação visuo-espacial também costuma ser afetada, podendo apresentar prosopagnosia, estereognosia (também avaliáveis no Mini-mental). **CONCLUSÃO: **Tendo-se em vista que os sinais e sintomas da DA são variados, principalmente nos estágios pré-clínicos, é de suma importância que os profissionais de saúde mantenham uma busca ativa no rastreio desses pacientes utilizando-se avaliações não só amnésticas como de outros domínios, como detalhado neste trabalho.
